Insurance and Payments

Thank you for choosing Oregon Eye Specialists to serve your eye care needs! Our primary mission is to deliver the best and most comprehensive care available. An important part of this mission is creating convenient ways to manage the cost of your care by offering several payment options. Our physicians are in-network providers for most plans from the major insurances and, as a courtesy to our patients, are happy to bill primary vision and medical carriers with whom we have contracts. Any fees not covered by insurance (co-pay, deductible, co-insurance or other uncovered amount) are due at the time of service. We accept cash, checks, Visa, MasterCard, Discover, American Express, and CareCredit.

Insurances accepted include, but are not limited to: Aetna, Cigna, EyeMed, Great West, Health Net, Lifewise, Medicare, ODS, PacifiCare, PacificSource, Providence Health Plans, Regence Blue Cross Blue Shield, United Healthcare and VSP. If you would like us to bill your insurance, please provide the details of your insurance plan with our staff when scheduling your appointment and bring your insurance card along with you at the time of your office visit. You are responsible for providing accurate and current information about any health or vision insurance that you carry. It is your responsibility to understand your insurance benefits (what your insurance plan will cover and pay for).

Some insurance carriers will not cover services unless a referral from the primary care physician is in place. Please contact your insurance company to determine if a referral is necessary, and in the event that one is, please request a referral from your primary care physician prior to your appointment. If a referral is required by your insurance plan, but one is not in place at the time of your appointment you will be responsible for payment in full for any products or services.

Minors receiving products or services must be accompanied by a parent or guardian. The parent or guardian will be responsible for payment at the time of the appointment.

Payment for products and services is required at the time of your appointment. Accounts with balances will be considered delinquent after 60 days and will incur a rebilling fee of $15. Accounts with balances after 90 days may be turned over to an outside collections agency.

At Oregon Eye Specialists we understand that unique financial circumstances can occur. If you are unable to pay your bill on time please contact our Patient Accounts department to make payment arrangements (503-244-1232).

Care Credit

Oregon Eye Specialists and The Sight Shop are now proudly accepting Care Credit. The two convenient plans offered by this medical credit card accepted in our practice for medical, surgical and optical purchases are:

  • 6-month deferred interest payment plan for purchases between $500-$1,499
  • 12-month deferred interest payment plan for purchases of $1,500 or more

Providing Your Social Security Number

We ask that you provide us with your full nine digit social security number. The full social security number is required to bill some insurances, Medicare, most workers compensations or Tricare to name a few. If your social security number is required to bill your insurance and you are not comfortable giving us your social security number, we will ask you to pay at the time of service. Your social security number is also used as a patient identifier by other healthcare organizations.  The social security number helps to distinguish your records from another patient with the same name and/or birthdate. Please know that your social security number is protected by state and federal privacy laws and we take our duty to protect your privacy very seriously.
